Final Exam Attendance Form - HIGH SCHOOL ONLY

 Please complete the following form if your child has 9 or fewer combined parent approved (P-PA) and unexcused (U-UN) absences this semester and you wish to excuse their attendance when they are not testing during final exams Monday, May 19th - Wednesday, December 20th.

You can check in Skyward to see your child's total absences. Please complete a form for each student attending West Lafayette Jr/Sr High School. Junior high students who are taking finals are expected to attend class when they are not testing. This form and the ability to leave is ONLY for high school students.
